Physician's Corner

Who participates?

Information about a person’s Alzheimer’s disease or related disorder is strictly confidential and should be entered into the database by or on behalf of the physician diagnosing or treating the person. There are no provisions at this time for patients or caregivers to submit information to the database. Patients or caregivers interested in having their information submitted to the West Virginia Alzheimer’s Disease Registry are encouraged to contact their physician and ask him or her to enter the patient’s data.

Supplying information to the West Virginia Alzheimer’s Disease Registry

Information about a person who is diagnosed by a physician as having Alzheimer’s disease or a related disorder may be entered into the database electronically through the physician portal at this website or by using a fillable form that can be downloaded from this website, filled in electronically or by hand  and returned by mail or fax.  The form may also be obtained by contacting the West Virginia Alzheimer’s Disease Registry.

Physician information

Physicians are asked to register with West Virginia Alzheimer’s Disease Registry to obtain a username and password in order to access a secure server on which patient data will be stored. Registration of physician information need only occur once. Information that is mailed to the West Virginia Alzheimer’s Disease Registry on paper forms will be entered into the database by West Virginia Alzheimer’s Disease Registry staff.
